The first section of this book is an alphabetical register of names of physicians and surgeons. The index includes name, address, birthplace, birth date, and page number of the affidavit. There is a column labeled "Authority for Practicing" that lists diplomas, certifications and endorsements. The second section of the book contains affidavits, which list the same information in paragraph format.
